The required form of certification was not confirmed first
Offices and matters set different certifier requirements, so a translation in another form is sent back for redoing. Confirm the conditions before producing the final version.
Parts of the document were left untranslated
Seals, signatures and end-notes are part of the original; dropping them makes the translation inconsistent with it.
Names are spelled differently from the identity document
When the translated name differs from the passport or ID, the office cannot link the document to the person, and the spelling must be corrected before re-filing.
A copy that the issuing office never certified was used
A self-made photocopy carries no verifiable signature or seal, so it cannot underpin a certified translation. Request a certified true copy from the issuing office first.
